Dear Referring Physician,
You may refer a patient to the Providence St. Mary Spine Center through any of the Spine Center Physicians, or by calling the main number 509-529-8969 or 877-344-1250. You also can refer by completeing the downloadable referral form.
Download, print, complete and fax the form to our office 509-529-6066. Our office assistant will be happy to make arrangements with your patient directly.
